US Warranty Claim Form

Please Note: This form is for United States customers only.

If you are experiencing an issue with one of our products, please complete the form below as accurately and thoroughly as possible. Providing detailed information will help us process your claim more efficiently and ensure a timely resolution.

If you purchased your product from an authorized dealer, you are required to submit proof of purchase. This may be a screenshot of your order confirmation or a receipt from the dealer. The proof of purchase must clearly display:

  • Your name

  • Date of purchase

Once we receive your claim, our team will review the details and respond within 10 business days. If additional information is needed, we will contact you using the details provided in your submission.

If you purchased direct from Jusn-Design.com please put that in the description box below. If you purchased from one of our dealers, please provide the name of the dealer where you purchased your Jusn-Design product.

Please put the name of the product you are having issues with.

If you purchased directly from Jusn-Design.com, please provide the order number which is in your original order confirmation email. If you purchased from one of our dealers, please put N/A.

Please provide us with the serial number of your product if you are having an issue with. The serial number is located on the right side of your product. If you didn't find it, please put "N/A"

Please give us a detailed description of what happened, how it happened or when you discovered the issue, and any other important information that may be needed in order to help you resolve the issue.

Please upload photos of the issue you are having. It is helpful if you provide us with both close up photos of the issue as well as photos farther away of the issue so that we can see where on your product the issue is located. If you purchased from a Jusn-Design dealer, please provide proof of purchase in the form of an original receipt or email of purchase that includes your name and the purchase date.
